Appraisal Authorization Form
I hereby acknowledge that I have authorized Iconic Mortgage Corp. to charge my credit card listed below,
for payment for the appraisal fee as indicated below. When my financial institution honors such charge(s),
this shall constitute my receipt for payment. I further acknowledge and agree to the following stipulations:
- I have authorized the charge to my Credit Card below for the appraisal fees incurred in connection with my loan application
- Any services corresponding to the below charges will be provided by a third-party vendor that is not affiliated with Iconic
Home Loan. - The appraisal fee is non-refundable.
- I am entitled to a complete copy of the appraisal.
- I understand that if my loan is declined or I withdraw my application, I may still be responsible for any charges ocurred.
